Elevated concentrations of lipids in the blood have been associated with an increased risk of cardiovascular morbidity and mortality ( 1). Monitoring cardiovascular disease risk and reducing the onset of cardiovascular events by therapeutic lifestyle changes and drug therapy has received increasing international attention since 1980 ( 2, 3). The detection, evaluation and treatment of hyperlipidemia are based on circulating blood lipid concentrations among which total cholesterol (TC) and triglycerides (TG). The classification of individuals based on lipid profile represents a general guidance for clinical decision making. The Accutrend Plus system offers high precision and measurement accuracy for all parameters and user-friendly operation. For measurement, a drop of blood is taken from the fingertip and applied to the respective test strip. The matching test strips, such as cholesterol test strips, are available separately in a practical box of 25. The Accutrend Plus System offers easy measurement of cholesterol, triglycerides and blood glucose on the spot and supports the early recognition of CVD risks as well as monitoring therapeutic measures in the doctors office or at the patients home. Accutrend Plus uses specially designed Accutrend Glucose, Accutrend Cholesterol, Accutrend Triglycerides and BM-Lactate superior quality test strips. The test strips contain a yellow protective mesh, impregnated with a surfactant (application field), a glass fiber fleece that works as a separating layer for blood cells), a reaction film in which the color formation takes place) and a bar code on the reverse side, which is read by the meter to confirm test strip identity, i.e. the lot number on the test strip is congruent with the stored code) Accutrend Plus lets you apply blood onto the strip inside or outside the meter. For example, when conducting multiple tests where cross contamination might be an issue, you can apply blood outside the device All venous and capillary blood lipid concentrations were determined in the morning after an over-night fasting period of at least eight hours. In each subject venous and capillary blood samples were collected at the same time. Supplementary capillary sampling was performed at two consecutive days following the first collection. Capillary TC and TG concentrations were determined with the Accutrend ® Plus using two drops of blood (15–40 μL) collected from different fingers, by using a lancing device (Accuchek ® Softclix ® Pro, Roche Diagnostics GmbH, Mannheim, Germany). The Accutrend ® Plus test is a capillary serum test. It is based on the retention of blood cells by filtration via a glass fibre fleece when a drop of blood is applied to the test strip. The enzymatic reaction that takes place in the underlying zone of the strip requires an adequate oxygen supply and results in the formation of a colored oxidation product. The reflectance of the strip (measured at 660 nm) is converted to concentration through a simple algorithm. The intra-assay precision of Accutrend ® Plus, as determined by the manufacturer, was 3.7% and 3.4% for TC and TG respectively. The inter-assay precision for Accutrend ® Plus determined with control solution by the manufacturer was lower than 5.0% for TC and 2.4% for TG. The control range of the Accutrend ® Plus has not changed compared to the older Accutrend devices (Accutrend GC, Accutrend GCT). Values outside the control range set up by the manufacturer (3.88–7.76 mmol/L for TC, and 0.80–6.86 mmol/L for TG) were excluded for statistical analysis. Normality of all data was verified using Kolmogorov-Smirnov goodness of fit tests. Hypercholesterolemia (plasma cholesterol concentration ≥5.2 mmol/L) is a risk factor for cardiovascular disease and stroke. Many different cholesterol self-tests are readily available at general stores, pharmacies and web shops. However, there is limited information on their analytical and diagnostic performance.The average plasma cholesterol concentration was 5.2 ± 1.2 mmol/L. The mean absolute relative difference (MARD) of the five cholesterol self-tests ranged from 6 ± 5% ( Accutrend Plus) to 20 ± 12% ( Mylan Mytest). The Accutrend Plus cholesterol meter showed the best diagnostic performance with a 92% sensitivity and 89% specificity. The Qucare and Mission 3-in-1 are able to measure HDL concentrations and can thus provide a cholesterol:HDL ratio. The Passing-Bablok regression analyses for the ratio showed poor performance in both self-tests ( Mission 3-in-1: y = 1.62x-1.20; Qucare: y = 0.61x + 1.75). The Accutrend Plus is unable to measure the plasma high-density lipoprotein concentration.Conclusions/interpretation: The Accutrend Plus cholesterol meter (Roche) had excellent diagnostic and analytic performance. However, several of the commercially-available self-tests had considerably poor accuracy and diagnostic performance and therefore do not meet the required qualifications, potentially leading to erroneous results. Better regulation, standardization and harmonization of cholesterol self-tests is warranted. The accuracy of the Accutrend ® Plus for measuring TC and TG was determined by comparing the results with those of the simultaneously sampled venous blood results using paired samples t-tests and Pearson’s correlation coefficients. The agreement between capillary Accutrend and reference method was assessed using Passing and Bablok regression and Bland and Altman analysis ( 19, 20). A difference between methods not exceeding 8.9% for TC and 15% for TG, as stated by NCEP recommendations, was regarded as clinically acceptable ( 3). Additionally the classification of the participants according to the NCEP for TC and TG was compared between both methods using Fisher’s exact tests and weighted kappa statistics (κ w). For the interpretation of the kappa values the following criteria were applied: κ> 0.80 “excellent agreement”, 0.80 ≤κ< 0.60 “substantial agreement”; 0.40 ≤κ≤ 0.60 “moderate agreement”, and κ< 0.40 “poor to fair agreement” ( 21). A significant number of primary care patients have dyslipidemia and hyperglycemia, representing key symptoms of metabolic syndrome associated with increased cardiovascular risk.
